Registration

Registration Process for Chappell Elementary School:
 
Registration is available by appointment only. Please schedule an appointment by emailing [email protected]. Prior to your appointment, please send over the following documents to help expedite the registration process:
  1. Child's birth certificate
  2. Two (2) proofs of current address. Please refer to the following list to see what we are able to accept:
    • Current utility bills (gas, electric, landline, water, cable, or internet)
    • Illinois Driver’s license or State of Illinois ID card
    • Deed or lease
    • Employee identification card
    • MediPlan/Medicaid Card
    • Court documents
    • Illinois Department of Public Aid card
    • Stamped United States Post Office change of address form
    • Illinois state aid check/social security check    
    • Other identification card issued by a federal or state agency or a foreign government consulate, such as a Matricula Consula
  3. A copy of parent/guardian's photo ID, such as a driver's license or passport. 
    • Please note, if your photo ID has your current address listed and is not expired, it can be used as a photo ID and a proof of address.
  4. FOR PRE-K ONLY: Proof of Income.
    1. Two consecutive, current paystubs OR
    2. 2025 Tax Return Form
  5. Medical, dental, or vision exam forms if they are complete. Forms are available on our website and are to be completed by medical professionals. These forms are due to Chappell by the time school starts.
  6. If applicable: Transfer form from previous CPS School / Pre-K placement receipt / ISBE form from previous out of district school.
