Parent's Illinois drivers license or Illinois State ID with a current address

Medical, dental, or vision exam forms if they are complete. Forms are available on our website and are to be completed by medical professionals. These forms are due to Chappell by the time school opens.
 
If applicable: Transfer form from previous CPS School / Pre-K placement receipt / ISBE form from previous out of district school. 
  1. To begin an electronic enrollment:
  2. Download and complete the enrollment form.
  3. Scan your completed enrollment forms, your license or state ID, and any medical or transfer forms (see above).
  4. Email all documents to [email protected]
  5. After the necessary documents have been received, we will contact parents by phone to make an appointment for parents to bring to school: original birth certificate;  2 original proofs of residency and (Pre-K only) proof of income (two consecutive paycheck stubs or W2 form) 
The Illinois State Board of Education requires parents provide a certified copy of a child's birth certificate within 30 days of enrollment. If a child was born in Cook County, specific information on obtaining a certified copy of the birth certificate can be found by clicking Birth Certificates.
